CDL Physicals & Medical Cards
The DOT/CDL physical is intended to aid in the safety of yourself and others. The FMCSA division of the DOT requires all commercial drivers licensed in the USA to get regular examinations every 2 years. Some companies required Non-DOT drivers to obtain medical cards.
QUEST/eScreen
We are a Quest Preferred Collection Site. We can perform all your drug, alcohol and hair follicle testing needs. Please be sure you have an email or print out with a barcode from the requesting agency.
Rapid COVID-19 Testing
We have Rapid COVID-19 Antigen Tests. You get your results in minutes. You will also get a document stating whether your are positive or negative as well as a receipt to submit to your insurance company. This is done with a nasal swab in one nostril and is 97% accurate. AT HOME TESTS are now available. $30 each or 2 for $50 or a box of 25 for $500.
Immigration
Let us take care of all your Immigration needs starting with creating a case at the lab, obtaining your DNA, contacting your loved one in your Country, making an appointment for obtianing their DNA and sending all the results to Immigration. Call us for a quote . Pricing is based on the amout of people and the Country. Please note that some Embassies are still closed due to COVID, and this will delay your case.

Keep left onto Avenue E. Follow Avenue E south for 2.5 miles until the stoplight at Avenue E & Broadway. Make a left onto Broadway. We are on your left side on Broadway between 5th & 6th Street.
From 440 North
Take the 7th Street exit and follow 7th street until Broadway. 7th Street & Broadway stoplight, make a right. We are on your left side on Broadway between 5th & 6th Street.
From 440 South
Take the left side exit for 5th Street. Follow 5th Street until the first stoplight at 5th & Broadway. Make a right at the light on to Broadway. We are on your right side on Broadway between 5th & 6th Street.
